CPP Reclaim for my deceased Father

Hi - my father died a couple of years ago and we discovered he'd be sold some card protection insurance by CPP - My mother and I wrote to them and cancelled the policy. My Mother has now received one of these letters and forms saying she is able to reclaim the monies. What I'm unsure about is what we should put on the form as to the reason for mis-selling - obviously my Father is sadly no longer here to ask and my Mother doesn't know, because she wasn't involved. I've looked at the different suggestions that you have posted on the site, but don't know which one to choose - can anyone help us - does it matter which one we put? Many thanks.
