Has anyone filled in a IB50 before?

Hi
From around January 2007 I have been on Incapacity Benefit and Income Support, due to Depression, etc, because I have been on the sick for around 6 months I have to fill an IB50 in(form from the jobcentreplus).
I just wondered if anyone else has had any experience of the form before, or has filled it in.

I have filled it in a couple of times in the past couple of years.
Get some help from someone like the Citizens Advice Bureau or Welfare Rights service. They know what they are doing and what to say how to say it etc
The first one of these that I did I didnt really know what it was and filled it in with no help and made a complete mess of it, plus combined with a bad medical examination(er) I failed. However I won my benefits back on appeal
The next time I had to fill it in I made sure that I got some help from the local CAB and they were great and very helpful. I still got called for a medical but I passed it this timeWeight Loss - 102lb0 -
As others have said, get some help to fill out this form, but in the meantime, this guide may help you:
