He'll need to send both parts in - to the fixed penalty office or the court, not to the DVLA.

It depends on the force, but you generally have to do the course (as opposed to booking it - there may be a deadline for booking it as well) within either 3 or 4 months of the offence itself. The reason is that in order to prosecute you there's a deadline of 6 months after the offence to lay information with the court (file charges to you and I), so if they gave you too long you could procrastinate about doing the course until it was too late to prosecute you... and thus avoid both the course and the points.
