What details needed to make a balance transfer?

Can anyone tell me what details of the card I am looking to clear with a BT I need to perform the BT?
As the original card was used only for a BT I destroyed the actual card. Not the 0% is up and I want to transfer the balance elsewhere do I need the details from the original card to transfer it elsewhere?
