    There should be a paper trail of your names, I do have a bit of a problem with some companies as the name I married under was not the name I was known as but could show birth cert, all the marriage certs, divorce paperwork etc and they could work it back. I did have a problem with my work after my last marriage, they wanted some info I was not willing to give them but they accepted my passport and a signed statement from my line manager agreeing it was me.

    You don't need a solicitor to change your name by deed poll. Here is a post I wrote on another thread which may help you.

    HOW TO CHANGE YOUR NAME BY DEED POLL


    Complete the form below, deleting the words in italics and inserting your new and old names accordingly. Sign the document using black ink, with both your new signature AND your old signature, where indicated. You must do this even if your signature remains the same.

    The person witnessing the name change must also sign the form in the space provided, using black ink. Note: the witness must actually witness you signing the Deed Poll in your old and new names.

    The witness can be anyone over the age of 18 provided that s/he is not directly related to the person changing his or her name. The witness must also add his or her name, address, and occupation where indicated.

    You will need several copies of the Deed Poll, and many organisations require a certified copy. Various professionals can certify copies for you, including solicitors, teachers, GP's. Make sure the wording is correct otherwise it will be rejected, if in doubt ask the organisation concerned for their preferred wording.

    Once the Deed Poll is completed it can be sent to the Passport Office, DVLA, and all companies and organisations, which hold your personal details, (Banks, Electricity/Gas/Water supplier, employer etc.); to provide evidence of your name change so that stored records can be changed to reflect the name change.

    By this Change of Name Deed, I the undersigned, [insert your new full name] of [insert your home Town, County & Country of residence]
    who was born at [insert your Town, County & Country of birth]
    on [insert your date of birth]

    DO HEREBY ABSOLUTELY RENOUNCE AND ABANDON the use of my former name [insert your old full name] and in lieu thereof assume and adopt the name [insert your new full name]

    In pursuance of such assumption I HEREBY DECLARE that I shall at all times hereafter in all records deeds and instruments in writing and in all actions and proceedings and upon all occasions whatsoever use the name [insert your new full name] as my full names and my family name and I HEREBY AUTHORISE AND REQUEST all persons to designate and address me by such names.

    In witness whereof I have hereunder signed my given name of [insert your new first and middle names if any] and my family name of [insert your new surname].

    Dated this ….[date eg first ]….day of………[month] …two thousand and fourteen

    I had a friend who reverted to her maiden name - it was actually very easy and involved professional organisations who are normally hard work over most things.. I think a lot of women change their names back these days, most companies etc must be used to it.

    That said, all the documents together shouldnt cause significant headaches to change it back

    If that's the name that was used on the marriage certificate then it's no different to anyone else reverting to their pre-marriage name.
