Help with filling out form to get direct payments into bank

We had our sons tribunal in December and have only just received the letter from DWP today to fill in for account details etc.
The start of the letter says when they say 'you' they are referring to the disabled person.
It asks about hospital stays (i assume it means for my son, not me)?
Then when it gets to the page for payments into an account it says - Part 1 - Customer's details. Is this me or my son?? It asks for name, address, NI number and date of birth. Then underneath is says - Part 2 - Apointee or representative's details. Do i fill this in here as me, or leave it blank?
I don't want to fill it in wrong or there will be even more of a delay. Oh and does anyone know how long it usually takes for them to sort out payments once this form has been returned?

Ok so the letter should have been addressed to you? Yes?
In which case just fill in your details etc. for the bank.
The hosptial side or any changes refers to the person/child elidigble for the benefit.
It will change at age 16 if he is still eligible (depending on capacity) it will either be paid to him directly or if he lacked capacity to manage his own affairs (this is a complex area) then you can be appointed an Appointee by the DWP. But at this moment in time it isn't relevant.
So you will be fine and just fill in your bank details. Sorry no idea how long it will take them to make the payment. Probably not too long. I would send the bank details back and get proof of postage (which is free) and keep a note of the date you returned the form.Think of all the beauty still left around you and be happy - Anne Frank :A0 -
I've just looked on the CAB website and it says the parent is the apointee if the child is under 16?0
