Starting a PPI claim

Hello!
I'm looking at starting several PPI claims and when I've read through Martin's FAQ the links to his template letters is broken for both, does anyone know where I can find them?
In regards to further down the line, for accounts that I no longer know the account number of, would a phone call to the company be able to help me find this out and then go from there?
I'll be honest I'm already finding this task a little bit daunting so I may well end up asking for help and advice on a lot of things that may already have been answered a thousand times else where. So i'll apologise in advance
