Trying to fill in questionnaire to reclaim ppi

I'm trying to fill in the ombudsman questionnaire as a first step to reclaiming credit card PPI for my husband.

I'm coming a little unstuck on some sections and the PPI section of this website says a guide to completing it has been put together in word format, but I can't figure out where the link to it is, can someone help?!
