Moving to a new area Jsa/hb advice please

Hi ive moved from doncaster to wakefield and i claim esa but my symptoms have gone now so need to go back to jsa.
How do i do this do i contact the doncaster jobcentre and tell them i wish to sign off esa or is there a nationwide number for esa and can i just start a new claim for jsa in wakefield.
Also regarding HB what do i need from my new landlord to send with the hb form is it just a tenancy agreement?
Any help would be great as ive never moved out of area before.
