Charge Certificate Received...but No PCN or NTO ever received

Hi
Received a Charge Certificate, dated 06 Jan 2014 asking me to pay £105.00.
It says I got a PCN on 02 Nov 2013, and a NTO on 4 Dec 2013, both of which I never got - had no ticket on my car and nothing has come in the post. The charge is for #27 - Parking next to a dropped footway, on Milford Place, Burley, Leeds.
I have been reading forums etc, and have decided based on everything I've read to do the following:
- Write a formal letter of appeal
- Include a witness statement confirming I never had a ticket, or received a letter
My question is:
1. Am I going about it the right way? Or should I ask for the ticket/photo evidence to be sent to me first?
2. Have Leeds City Council broken the following 'There has been a procedural impropriety by the council', by not sending me the proper documentation? And therefore is that my grounds for appeal? On all the info I've read, I can't find any guidance on the timeframes they have to send info to you in.
3. The £35 fine I wouldn't be too bothered about paying if it turns out I shouldn't have parked there (it's the road outside my gym where I park most days, so I'm unsure exactly where I parked without lookign at the photos), but if I could use the fact that they have done the above to get me out of paying it at all, I would be happy with that too!
Any advice would be most helpful!
