Template for finding out if I had PPI?

Hi all,
I've been trying to find a template letter but haven't been successful. Basically I have no idea if I had PPI on a variety of different mortgages/ loans / credit cards etc. and I haven't been great at keeping paperwork until recently.
I have done a free Experian report and now have a list of previous accounts, and addresses to write to, but no idea what to say. All I can find on the main MSE site is a suggestion to either call them or get in touch via mail. I'd like to mail, as I tend to get flustered easily on the phone! But I could do with a template letter if anyone has one please. Do I simply need to ask them 'did I have PPI on this loan/cc' ? And are they obliged to tell me?
Huge thanks in advance if you can help

Remember, though, that it's not enough simply to have PPI. In order for it to be refunded it has to have been mis-sold to you. In your case, it's difficult to see how you can have any valid complaint reasons as you don't even know if you had any PPI.
It's a myth fostered by claims companies that Banks randomly applied PPI without the knowledge and permission of customers..0 -

Mortgages are easy. In most cases, they are standalone direct debits. That is also one of the reasons why most mortgage PPI complaints fail.I am an Independent Financial Adviser (IFA).
