electoral roll, name differences

Equifax thinks I'm not on the electoral roll. When I look at my credit file, it reads my mum, dad, sister, "John Smith" (me) and "John R Smith" (also me).
I must have applied for something in that name once.
How do I fix this? Is a correction notice my only option, or can I go to the credit agency and get it rectified?
