Not really sure on what advice anyone can give as his school history is disgracefull. Any comments appreciated..

LEGAL REQUIREMENTS
All schools from the [text removed by MSE Forum Team] Education Authority have failed to comply with legal requirements . Resulting in lack of support and provision.This has had a detrimental effect on ... education.

‘Maladministration’ has occurred where the school has:
Not followed its own rules or the law - Policy on SEN
Broken its own promises - " "
Failed to provide information or given the wrong information - IEP'S and Reviews
Failed to provide SEN, SAP support and provision to commensurate his needs
Differentiation of learning within the Curriculum have not been put into place for his learning needs.

2000 A routine hearing examination revealed (text removed by MSE Forum Team) had been unable to hear clearly until he was 2 1/2 yrs old, when he had grommets inserted due to having glue ear.

*Jan 01 Failed his 2 1/2 yr language screening. Only saying mainly 2 word sentances eg its burny, whos that and play bikes.

April 01 Speaking, Listening, Reading & Writing - Developing competence

July 01 Same tests as above 3 months later - No observable evidence of competence

April 01 Numbers, Shapes, Space and Measures - No observable evidence of competence

July 01 Same tests as above - Barely developed competence.

June 03 Reception Class- First ever SEN meeting, Teacher comments that she "has concerns"

June 23 03 5yrs 2mth, Hearing problem on routine check , I will question at the hospital and school will re check in 4 weeks and monitor

Nov 03 SIL Test 8/24, Age 5 1/5 he first presented with behavioural and concentration issues.
Cannot retain information, to monitor behaviour, needs to finish set tasks.
To improve listening skills, concentration and behaviour.

Feb 04 Problems with listening skills, still lacks ability to concentrate, behaviour issues. Clinical psycologist arranged for end of March.
P.I.T involvement - assessment taking place

Feb 23 04 Speech and Language Therapist reports, sat on carpet just looking at teacher?

8 months after hearing problem detected, he wasn’t re-checked or monitored. Three comments noted in relation to his listening. I am beginning to wonder if he was partially deaf at this point.

April 04 At age 6, (text removed by MSE Forum Team) was placed in School Action Plus, where he has been all the way through school.

June 29 04 Bizarre behaviour, easily distressed,obsessive and possessive behaviour. School requested appointment with Educational Psychologist and stressed it needs to be kept due to concerns from school ?

Nov 04 Behaviour, emotional and Social difficulties. Not recognising letters, not retaining information. Listen to be able to respond,
Educational Psychologist booked for Dec 04. EP request for Statement Assesment ?


April 05 Sounds need work, letter formation poor, spelling sometimes bizarre, often puts ends of words? Observed by Speech and Language Therapist for behaviour support.

April 16th 05 (text removed by MSE Forum Team) for assesment, 4 months previous, before he left and to discuss findings at next multi agency meeting."

Under Paediatrician (Health Centre) & Psychiatrist [Text removed by MSE Forum Team] Behaviour, Anxiety and concentration.

Yr 3 July 05 No IEP provided.

Still behind, he comments about his father with a violent tone. (not seen for 4 1/2 yrs)

Nov 05 Speech and Language Therapist Report, recommends;

- Needs prompting to stay on task
- TROG Test Score significantly below for age
- Instructions need repeating to understand.
- Hard to hold information to complete the task.
- RENFREW Word Finding and Vocab Test,Vocab much smaller for age.
- Pronounces inaccurately- eg scarefrow instead of scarecrow
- Word finding difficulty, when [personal details removed by MSE Forum Team] accesses his memory for a word, information is not stored accurately and is lost, so says it wrong or one similar.
- Semantic Skills are weak. Difficult to infer information
- Inappropriate topics of conversation often relating to death
- Finds hard making eye contact with who he is talking to. ASD Trait.
- Story plans, mind maps and comic strips are also aids for ASD pupils?
- Finds Phonological awareness tasks quite hard, skills inconsistent also inability to identify the number of syllables in a word.

Nov 05 Seen by Paediatrician, could not detect abnormal thought perceptions ? Needs visual clues to understand.

With the Speech and Language Therapist's recommendations in place, [personal details removed by MSE Forum Team] began showing signs of improvement;

Jan 06 Moved school

(Text removed by MSE Forum Team) regarding results of his assessment, 9 months ago and was now told;
"No report was made or outcome ?? " although April 05 Review said "saw Educational Psychologist (text removed by MSE Forum Team) at ..... before ....left and to discuss findings at next meeting." ?

Yr 4 Oct 06 Uses a beanbag or stress ball to help sit still at carpet time. Lacks concentration.
His teacher told me that she had no help and couldn't keep explaining when she had a class to take ?"

Feb 07 The lack of support began to show itself with the decline in his behaviour at school and at home and his enthusiasm to learn.

July 07 Comment saying ... needs to realise discipline is important to make progress in school.

Oct 07 Wont work at home so making little progress, attitude to learning and little enthusiasm.

April 08 Moved from .......School Records did not arrive
Concentrates more when working independently, tends to rush.

Yr 5 Sept 08 Tends to rush and not completely listen

Yr 6 Jan 09 Anger and tears through frustration with maths.

June 09 ...... to inform ..... of past reviews and abilities.
Support and carry on with his SAP. Academic interventions and SEN structures required in Maths and Literacy to further his confidence..

Yr 6 he regulary attends after school booster classes and he excelled his predicted grades.

Initial assessments shown .... to be above National Standard( see Progress report below) and with ....... failing to send his records at this crucial time, they were not aware of his history nor the advice passed on.

.......... for not transferring his records within the 15 days and also ............ for not identifying the problem.

"Considering schools have a duty to make sure records have been transferred within 15 days I cannot understand how this could happen and while it was ............s responsability to send them, Why MCMA did not investigate why they had not arrived?"

Couple of anger outbursts and fights and had a time out card, by Year 8 he had calmed down.

Dec 09 Yr 7

First Progress Report from MCMA, predicted he was a GCSE Grade A-C student.

Maths working at 4.2 Expected end of year 7 4.9 Target end of KS3 5.8

Due to no support or learning strategies put into place his levels continued to fall. If appropriate staff within his teaching groups, adapted and modified their lesson in order to meet his needs I am convinced this would not of been the case?

June 11 During a meeting with [personal details removed by MSE Forum Team] 's Form Tutor, I asked about the SEN Funding and that The Code of Practice states all SEN pupils are entitled to help and schools have a legal duty to do this. I was quite alarmed when he told me it was not for each individual but comes in a lump sum and that all the help goes to those who are statement.?

He was a pleasant member of the form who was always ready for the school day.

July 2011 left MCMA. To prevent any problems for [personal details removed by MSE Forum Team] 's transition to ....... I took all of [personal details removed by MSE Forum Team] 's reports etc to SENCO, she kept them for 10 weeks to copy documents needed and to put into place his SEN provisions.
Yr 9 Sept 2011 started at .........- (Oct 2013 It has now come to my knolege his records did not arrive from MCMA)

Comments from his Head of Year, Form Tutor and subject teachers now tell me
(Oct 2013) none had been notified of [personal details removed by MSE Forum Team] 's history or provisions.

Underachieving, behaviour, concentration and focus concerns have consistently been reported to me, whereas nothing has been put into place support wise or his work differentiated.

Feb 12 Form Tutor (text removed by MSE Forum Team) ,Comments; attitude had become worse, rude and defiant. A deterioration in behaviour, rebelling against practically everything. He advised (text removed by MSE Forum Team) about choosing his friends carefully and not with those who he knows are going to get him into trouble.

July 12 Refused to go on the school trip to Alton Towers beacause of his behaviour

Concerns about behaviour issues and consistency about attitude. Unfocused in lessons and to improve concentration.

May 13 Maths workbooks were arranged to be sent home but they never arrived.

Yr 11 Sept 13 Behaviour has escalated, In the first couple of weeks he has began to walk out of classes, truant, refuses to do detentions, climbed a 10ft fence rather than go in the SIR, swore and made a threatening remark to a teacher. Resulting in a Fixed exclusion for 2 days...No work sent home.

Since (text removed by MSE Forum Team) started at ............ his Levels have continued to decline, I am frequently informed by his HOY and subject teachers that he is underachieving. None appear to be aware of [personal details removed by MSE Forum Team] 's SEN situation so the correct support or understanding of his behaviour is unknown.

(Text removed by MSE Forum Team) has proved with the correct support and guidance in place he is an achiever. The A-C GCSE Levels he is capable of, are still within reach, but I due to his attitude to learning this will not be the case.

He is refusing to revise and go to after school clubs. [personal details removed by MSE Forum Team] has lost total confidance in the system and himself, showning itself by displaying such rebelious behaviour during secondary education.

What support is in place for him?
No IEP'S have ever been sighted by myself and unaware if any exist?
Review meetings have only ever taken place after I have contacted SENCO with concerns.

Spoke to (text removed by MSE Forum Team), who confirmed (text removed by MSE Forum Team) was registered School Action Plus in Jan 2012, arranged to pick up copies of IEP'S, and Reviews.

(Text removed by MSE Forum Team) rang me and I asked her what will be put in place from now until exams, her reply was some one may be brought in to sit with [personal details removed by MSE Forum Team] in a room and have questions read to him ????
Collected 7 IEP's dated Jan 2012, July 2012, July 2012, Sept 2012, May 2013, 8 July 2013, Sep 2013, and Oct 2013 and 2 Reviews.

It was clear to see all of the paperwork had been done that day as apart from a couple of different comments, every one was the same.

7 Nov 13 see letter dated the same. 5 day Fixed Exclusion again no work sent home. Told to go online but dont have any IT at the moment.

13 Nov 13 Meeting with (text removed by MSE Forum Team), confirmed neither she or his subject teachers were aware of his SEN?

14 Nov 13 Meeting with Head and SENCO. My queries went unanswered. ( See meeting docs.google.com/document/Qu’s for meeting13.11 .13)

I Suggested 1:1 tuition recommended on DfE, instead (text removed by MSE Forum Team) an Academic mentor was arranged. According to July 2012 review this was supposed to happen then?
This broke down after 2 days as (text removed by MSE Forum Team) didnt want to be placed in a room all day and wasnt benefiting from the mentor.

Why she told (text removed by MSE Forum Team) when he was supposed to be oblivious to the fact to get a true assesment?
1pm I had appointment with her and (text removed by MSE Forum Team) turned up so joined in the meeting,

Discussed was him being able to go to a referral room at times of stress.
Not getting in trouble on corridors when has a pass.
All teachers to be notified of his SEN, When work is explained [personal details removed by MSE Forum Team] cannot absorb the instructions so is left not knowing what to do? Teachers should of known this, preventing him ‘mithering’ teachers for help and seen as being nuisance.

Due to truenting he was told he was being put in SIR today? An argument lead to him swearing at her and her screaming in his face.
As his Pastoral Officer she is aware exactly what ‘presses [personal details removed by MSE Forum Team] ’s buttons’ so is not the ideal way to deal with him, knowing how he could react.

I was called into school to escourt him home due to violent behaviour and verbal abuse to (text removed by MSE Forum Team) .

12 Dec 13 Although most of day not present in school and sent home , his 2 day exclusion did not start until today and for tomorrow,so in total he was actually off 3 days.

    There is a HUGE amount of indentifying information which needs to be taken off the board NOW for fear of identifying you, the teachers and your son.

    If your son is badly affected, what is he doing in mainstream school, what are you doing about all this, what does your son's psychiatrist/therapist say and why are you identifying your son as 'special' to the whole www?

    I agree with Ginger nutty. There's far too much info. there. It's great that you have it all but it shouldn't really be shared on an internet forum in such detail.
    So basically you son is in year 10 (or 11?). He had behavioural issues which are detoriorating. You think he has ASD? He has hearing issues as well. He has SEN but half the teachers and his Head of Year don't know he has SEN?
    The school appears to be letting your son down badly from what I can see. Do you have any idea what the previous school were doing that actually worked for him before he was transferred to this new school?
    Have you written to the chair of governers?
    Have you go a parents partnership or similar in your area?
    Have you spoken to your family doctor and got any tests/referrals done? Has he actually been assessed by CAHMS?/Got any formal diagnosis yet?

    There doesn't seem to be anything about the family's role in all this and what strategies are being used in the home and what help is being sought outside school.

    There is far too much personal information on here. I do not agree that you should be naming the educational professionals in question here as your account is very one-sided and as other posters have remarked, seems to completely leave out anything family-related.

    What are you doing for your son? The onus cannot purely be on the school to address all of his needs. You need to be taking him to medical professionals for diagnoses and you need to be writing to the LEA to request that he is considered for a statement.

    Whatever the situation it's entirely unfair of you to be naming all of these people on a forum and I strongly suggest you remove all personal information and names from your post.

    When did he receive a statement and how many hours is his statement for/is it for specific subjects? I only asked as I couldn't see him being given a statement, but school action plus.

    Legally, only statement students need support and they are entitled to x amount of hours that is set to them, those who are school action or school action plus do attract some extra funds, however this doesn't have to be used on an individual basis by say employing a teaching assistant for that child or employing a tutor, but things like different resources, e.g. books aimed at those with dyslexia etc.

    How are you managing his behaviour at home? How do you further support the school when he is excluded (from number of exclusions I am suprised he hasn't been expelled), you say you had no IT when he was excluded, did you call the school immediately to request paper work?

    When did you refer him to be assessed for his SEN, did you give permission for this to be passed on to the school, you would be surprised how many parents forget to do this, as without the parents permission the school cannot access any of the information.

    What school work does he do with you outside of school?

    Where do you live? The assessment levels you show aren't used in England or Wales.

    OP whilst I have sympathy for your concerns, you seem to be looking backwards rather than forwards.

    This month your son will no doubt be sitting his mock GCSE exams, handing in final coursework and taking GCSE modules which count toward his exams. You really need to be working with the school to help him get through this very intensive time.

    What sanctions did you put in place at home when he was excluded? He should have been revising or finishing coursework during this time.

    In six months school and exams will be over and he will be about to start the next stage of his life. What will he be doing and how can you help him?

    Thank you for all your replies. Unless you can see details, I cannot, I believed the mods removed any incriminating evidance.
    I only receintly for a CAF meeting put this history together, hoping all comments made by the proffesionals not me, may of helped them in a diagnosis. It was his SENCO who receintly referred him to CAMHS due to the asd traits he was showing. They now are looking at ASD/ADHD and ODD.
    I have placed my trust in the education and health proffesionals and feel , no has , been let down every step of the way.
    His Levels I have taken from reports from school so if they are not used in England, you tell me ?
    His home life, brought up single handed by myself from the age of 2 1/2, have helped him in every way, sought and taken advice from reliable parenting sites, parent partnership etc I dont no what you want me to say with regards to his home life, I suspect my replies and from proffesionals in either field and are in their defence redirecting the reasons for the decline in his behaviour and levels to my direction.
    I dont no what I responce I expected when I made this post out of sheer desperation and frankly wish I hadnt. All questions asked, the answers are in my post.

    I agree with lulu, it is time to look forward. He has 5 months until he finishes school. In that time what help can you give him at home to assist with his studies? What plans have been made for his next stage?
