Does the PPC pay for the POPLA code when they generate it or when it’s used by the RK or driver?

When it is used, I am sure. Lots of victims don't realise the importance and don't appeal to POPLA. PPCs don't pay for all those as they never reach POPLA.PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).

Strictly speaking, neither. If the RK or driver submits an appeal using the POPLA code, the PPC only incurs the £27+VAT charge if they contest the appeal. I have been providing assistance, including Lay Representation at Court hearings (current score: won 57, lost 14), to defendants in parking cases for over 5 years.
