Help needed with RM Claim Form please

Got an INR case with ebay at present and about to fill in a Royal Mail Claim Form.
I've read section 2 which quotes the information needed and have some questions:
1) The Paypal or credit card statement needed - does this mean the Transaction Details Page from Paypal for the sale (which also shows the refund)?
1a) Do I need to 'hide' any personal details from this eg. Buyers ID, Buyers email address, my email address when I've printed them out?
2) Section 2 just asks for the name and address of Sender and Recipient but Section 6 is also asking for date of birth, phone numbers and email addresses.
Do I have to provide these extra details? And why d.o.b.?
The parcel in question was well packaged, sent normal 2nd class with certificate of postage. I do believe that the buyer is being genuine.

Can't help with question 1) but with regards to question 2) I have never put DOB/email address/phone numbers for sender/recipient on the RM claim forms. They haven't sent one back yet asking for this information.0

Hi there,
I do a fair few of these and they are very annoying. I fill out the form with as much information as possible, I don't put in the buyers DOB or email address, just their address.
I always send a print of the Paypal transaction and also print out my messages to and from the supplier regarding the item going missing (they don't ask for this so it isn't essential).
Just a note, I would photocopy your proof of postage before sending it off, they lost my claim form once with the proof of postage.0 -
Thanks for the replies. Just to clarify, it only asks for the senders d.o.b.
I will be scanning the certificate of postage and sending a copy of the messages from the buyer, thanks.
The Paypal transaction page includes both mine and the buyers email addresses. I think I would prefer to black these out if not required?

Quick Update:
After providing all relevant details and full descriptions I received a letter from Royal Mail saying they were looking into it. Earlier this week I got a letter from them saying they cannot identify any reason why this problem has occurred and quoting actions they have taken.
They enclosed a book of 12 first class stamps as a refund of the postage I paid. The claim was for £7.50 (inclusive of postage costs) so it is near enough compensation BUT I would have preferred money back instead.
Makes me wonder if I would still have just got 12 x stamps if the claim was a higher amount instead of proper compensation?0
