Social worker refusing urgent care assessment - UPDATE - advice please

I find that whenever DH has a psychogeriatrician's appointment, when they assess the progression of his Alzheimer's, that I am asked to talk about things that have changed since the last time.
And of course, nothing's improved! DH will also say "no, I didn't do/say that, you're making it up!", to the first negative comment. So I now say "well, I don't want to distress DH by listing the plethora of things that I should ..." and they usually nod, and once they've started on the first few items in the test, it all becomes apparent.
When I was teaching, I used to make myself hoarse by saying "make sure you read your exam paper through twice!" And because people feel uncomfortable if they're being watched, they rarely read the small print on a document they have to sign there and then.
Have a good night's sleep (if you can), and remember, it's only a few weeks since you first came on MSE asking for help - look what's happened since! :beer:
