JSA stopped even though I filled out holiday form?

It's just a guess but it's possible 'failed to sign' (FTS) action has been taken by someone in the JC not realising that the holiday form has been done.
The presence of the form in the building doesn't necessarily mean it's obvious when someone is working on the claim.
A rapid re-claim (RRC) may create further problems. You really need to establish with them what happened. By the time you make contact, they may already have discovered it's gone wrong.
If so, that will be better than attempting RRC and possible appeal.
Something else to be prepared for: Even if it was a simple error that will be corrected, with JSA claim reinstated, just as you have received an end of claim notice, so will the HB office.
There's a strong possibility that this will interrupt HB even if JSA is quickly resolved.
HB staff do have access to JSA computer records, so the minute you know the problem has been resolved you need to make sure the HB office knows. Otherwise you may end up with the usual HB start up delays.
