Claire, sit and write down the skills you have, and don't worry too much about not having continuous employment. What can you do?
Work well in a team? Contribute to projects? Work to deadlines? Remain focused? Good with customers? Good listener? There are loads of things you are probably using now to find a job - for example resourcefulness as you found this website and posted on it. In terms of getting more confidence, try little things like asking your WP advisor for help with things, and build up from there. Try some group interviews and just do them for practise (if you are able to get them) and don't worry about coming across as a prat - I used to worry about that but now I've done so many things that used to embarrass me I am not so bothered. Are there any other things you can go that help push your boundaries, for example I do dance classes (despite being rather podgy and unfit) simply so I constantly push my boundaries of embarrassment and I stop myself from saying I can't do this. Ask your WP for help with any courses such as CVs and interview, they must do ones for confidence, or could suggest other places where you could do them. Also consider talking to your GP, they can put you on a CBT course for anxiety and confidence. You would be amazed how many people are currently on NHS courses for these things. Maybe they could even recommend a counsellor to help you have the confidence you need. YOU ARE ABSOLUTELY NOT THE ONLY ONE TO FEEL LIKE THIS.
