ESA assistance please

First of all hello all and thankyou for this informative forum.
I have been on incapacity benefit for a few years and was put onto ESA last year.
I went to my atos medical and was initially deemed fit for work, but after appeal had the decision overturned. during and after my appeal I was put onto ESA (contribution based) for 365 days.
I have been receiving various letters over the last few months saying my benefit stops on 08.10.13 and to make a new claim, but due to my mental health have been unable to put anything in motion until now.
I am finding it extremely confusing filling out this new form (ESA3) and find a lot of questions pointless and irrelevant.
As my appeal was successful and I have been deemed unfit for work I am finding it hard to understand why I have to fill in another form to make a claim.
Why cant they just transfer me from ESA CB to ESA IB ...
Also I should add my partner is self employed and works approx 16 - 20 hrs a week.
Any help or advice would be much appreciated.

You have to fill in the form so they can assess the household income and savings before seeing if you are eligible for income based. It's not a new claim at all but just looking at finances.Lost my soulmate so life is empty.
