Consent order & D81

Hi
Ive been given a D81 from my ex who I left in 2011 and divorced in 2012, which he says I've to fill in,sign and give him back so he can do his part and pass it back to his solicitor. The thing is, we've already agreed on a settlement figure which is less than half the value of the property based on valuations that he had done and I'm concerned what will happen with the information when I fill in all my financial details. I work full time but being made redundant in dec and have a pension, whereas he's been on Incapacity benefit for 25 years because of a bad back. To cut a long story short, on paper it looks like he's getting a bad deal but in reality, he hasn't contributed to the mortgage until Oct last year when I stopped paying it. His gf who works moved in last month and its her income that's secured the loan to buy me out.
If I don't disclose the information and just put on that we've mutually agreed a figure can his solicitor still get a consent order completed?
I appreciate that depending on how this is read, people will have different opinions but I couldn't possibly post the whole story. Any advice on this would be appreciated
