Worked ok in Chrome. It does say "FORM WILL APPEAR BELOW ONCE YOU HAVE LIKED THE FACEBOOK PAGE AND ARE SIGNED IN." - I was already signed into Facebook and had previously liked their page from some other comp0

I had to unlike (click the Like button then the x) then Like again to get the form to appearBest Comp wins in 25 years of comping.
