1. Firstly grab a National Lottery Lotto ticket valid for any Lotto draw between Saturday September 28 and Saturday October 19, 2013.
2. Then you must simply crack the three codes that will be printed inside The Big Lotto Giveaway pull-out (from your Daily Mirror on Tuesday October 8, 2013) in order to discover the three correct answers as part of our Big Lotto Giveaway game.
3. Once you have cracked the three codes input your answers along with your unique ticket number from your National Lottery Lotto ticket into the fields in the form below. To see where your unique ticket number is, CLICK HERE .
4. Please make sure that you complete all of the required fields in the form below and that you keep your Lotto ticket safe as you will need to present it if you are a winner.

You can enter for each lottery ticket number you have, so if you have 3 you can enter 3 times etc.

Also, it says that Lotto tickets much be retained but what if you buy your tickets online? :think:

ETA: Sorry, just noticed you copied the T&Cs that says about being able to enter per lottery ticket number. I blame baby brain!
