Help with power of attorney online doc

My OH is applying to get power of attorney for his mother who is now in nursing home and she wants him to be in charge of her affairs. now I think I am being really stupid here but we have been on the gov.uk site to fill in the power of attorney doc and I am stuck at the first post!!!!!! it starts off with filling in details about yourself, now who are they referring to my MIL or my OH?? as 2nd section is you have to fill in details of who you want as your donor and says The 'donor' is the person making this LPA and appointing other people to make decisions on their behalf. The donor: then the next section says who do you want as your attorney??
so I am confused now basically I need to know
1 who's details do you put in on the first page
2 who is the donor
3 Who is attorney

Assuming England/Wales, is this for a health/welfare LPA, or a property/financial affairs LPA, or are you doing both?
