Defaults on credit report

OK .... I'll make this as brief as I can. Am looking for some advice before starting to tackle this problem.

It's taken 2 years but finally got the mother in law sober and ready to face up to her problems. She's been in debt most of her life but here are the current problems:

1. checked her credit file as she really needs to move and she had a credit card passed to a DCA many years ago. She's been paying £50 a month (with about another 10 years to go I think). Experian and CallCredit show this debt in existence twice - once with the original credit card company (showing full default balance) and once with the DCA (a reducing balance is reflected every month). She pays the DCA every month not the original company.
QUESTION 1 - is this right or should one of these entries be removed? They both hold the same card number, same original balance and same date of default (2008).
QUESTION 2 - despite the monthly payment and reducing balance, the DCA is registering a default every month against the account - is this correct?
QUESTION 3 - what happens 6 years after the date of default as she is still making payments? Will this default then stay on her account until she has finished paying, will it stay on her account after she's finished paying +6 years or will it drop off next year?

2. She has a CCJ from 2009 from unpaid utility bills. We settled that for her back in 2009. Will this then disappear from her credit file in 2015? How will it affect her ability to remortgage in the meantime - it was fully settled within a month of the CCJ (when she finally told us about it!)

Any advice would be gratefully received so we can judge the best time to help her move into a property she can manage. Thank you.

    If the CCJ was settled within one month then it should have been removed from here report back then. Did you provide the court with evidence it was settled?

    Ooo, fantastic thanks. So if I am understanding correctly (as it seems a bit of a minefield....).

    Q1: we need to get one of the entries removed. Would we write to the original lender, DCA or CRA to do that?
    Q2 and 3: so it's not really a default - just a status update and it will still drop off next year then even though there will still be an o/s balance?

    Re CCJ, we paid it as soon as she told us about it. I guess she may have waited a while before she told us - I just assumed she told us immediately. I will have to find everything she gave me to confirm dates. Regardless, we didn't contact the court - we just paid the company she owed. But thank you - will look into it. If she's stuck with it, she's stuck with it. If she did tell us promptly then looks like we can do something about that.
