Hunting around for something else, I found this!
You can now complete a lasting power of attorney online

The Office of the Public Guardian has built a digital tool which will make the process of creating and registering an LPA simpler, clearer and faster. The digital LPA tool offers:
  • Step by step guidance: it breaks the process down into small, logical steps.
  • Fewer errors: it checks for errors and prevents you from going further until you have corrected them.
  • Help where it’s needed: it provides relevant guidance at the point you need it – with further help available at the click of a mouse.
  • Fewer forms to complete: it automatically chooses and fills in all the forms you need with the information you provide. All you have to do then is print and sign them.
  • Online secure payment: there’s an application fee for each LPA you register. Our online payment partner will ensure your payment is safe and secure.
  • Secure personal information: all the information you enter is safeguarded on protected government servers.
You’ll need to print out the completed LPA forms so that all the relevant people involved can sign it, but the tool will help make the whole process quicker and easier.

No time like the present, I have started to create one and it seems quite straightforward with good links to help at regular intervals.

The only thing I'm a bit frustrated by is that you can't seem to skip a step and come back to it later. Because I've only just started thinking about this, I don't know who to use for my certificate provider, and I'll obviously have to ask them first.

    I saw that too, Savvy_Sue.

I've opted to do hand written copies as I think it will help my Mum understand things better and the guides that they send for each LPoA are pretty comprehensive.

In fact, I sent for copies a while ago and the completion was shelved for family reasons but things have now changed and Mum wants to go ahead again.

So, because I wasn't sure that the original forms I'd got were still up-to-date (they were sent to us over a year ago) I've used them as 'testers', filling in details and writing comments.
It's helped me get my head round the process.

We've opted to use Mum's GP as the sole Certificate Provider and her 2 sisters as 'people to be told'.
He knows Mum well and also knows what my sister & I do for her.
It will cost us for the GP to do it but we feel it puts things on a more official level (we have an estranged close family member).

Yes, there is also an option for just downloading the blank forms, but I did find the online process helpful in terms of establishing what we needed to know.

DH says I can set one up for him as well.

We do have an EPA which we sorted just before they stopped being possible, but I looked at it the other day and suspect there's a problem with the witnessing, plus only our eldest was old enough to hold PofA for us and now we can use the younger two as alternates, and do the Health one as well.

  • I have just completed both LPAs for my mum - the health and welfare and the property and financial. The best way to use this is to ensure you have ALL your information to hand as it will not let you continue to next page if current page not correctly filled. It is really easy to use and I found it much much less confusing that the lengthy guides and endless amount of papers. The online tool automatically crosses out any boxes you do not need to fill in and if you want to do both parts once you have completed one part it remembers the information you have and assuming you are using the same people you can choose from a drop down list which then auto inserts all their relevant information. It gives you a SHORT SIMPLE guide to signing:my mum chose her friend of 40+ years as her certificate provider ( so no hefty doctors costs), one brother who lives away as the person to inform and my other brother and myself as attorneys with my 2 daughters as replacement attorneys. All done within 2 days and sent off for registration. My mum is happy and understood it all. I had some questions and phoned the office of the public guardian - really helpful advice, very friendly. Its the best way to do these forms and the most simplified way.
