Complaint contact lens solution/access to GP/optician records time question

Hi,
6 weeks ago bought expensive hard contact lenses and had the best vision of my life!
- but
2 weeks later purchased new bottle of contact lens solution (same manufacturer as lenses).
Allergic reaction eyes streamed, constantly -
Emailed complaint to tmanufacturer
they want bottle returned and have sent a form to ask for access to GP and optician notes.
My Q. is they have sent an Authorization For the Release of Protected Health Information. The last para says:
' Expiration; This will expire.
Upon completion of the requested disclosure
Six months from date of this authorisation form
On .... blank ... date
one year from the date of this authorisation form.
I understand that this authorisation shall become effective immediately and unless otherwise revoked by me in writing shall expire as indicated above. I further understand that when my PHI is disclosed pursuant to this authorisatin it may be subject to redisclosure by the persons authorised to receive my PHI. I understand that certain records may be protected by federal and state confidentiality laws that prohibit the recipient from making further disclosures without my written authorisation.'
Sorry that was long.
So I just want to authorise the trip to the GP and Optician about my eyes and nothing else, and specifically about this incident. Should I just tick 'Upon completion of the requested dksclosure/?

Is this an american company?
Data protection in the USA is massively more lax than it is here in the UK. At the end of the day I doubt it really matters if your GP report saying you;ve no known alleges prior to this event goes global but always worth a slight second of thought.
But yes, there is no need for them to have continued access to your records so the first one is fine. Your GP may demand a second form however if they come back with additional questions/ queries based on the content of the initial response given the consent will have expired.0
