I have rang up for one of these forms 3 times in a month.

They say that they are waiting for the printers!

This is the Welsh section.

ARE they just not sending out these forms.

Also how low does your income need to be to receive this credit.

You should be able to get one from an optician, job centre plus or doctors surgery, my son got his from his optician when he started university. I'm not sure what your income level has to be, my son just receives his student loan and about £150 from me, so around £400 a month really and he received a £40 refund when he purchased his last pair of glasses and he got a free eye test.

I live in England and picked up one from the Optician last week. You should see if any local dentists, opticians, pharmacies or Drs surgeries have a copy.
You are assessed on individual circumstances so there is no one size fits all figure i'm afraid.
