    Restrictive isolating valves (those are not full bore to my mind). Not tightened up enough and pressure build-up pops the pipe out.

    OP: perhaps you have already seen this but have a boo here.
    Alternatively, good advice from Speedfit.

    I've never had a problem with speedfit. I think your dodgy pipe cutting is likely the culprit.
