Washing machine y-piece leak

I had a leak from the Y-piece for the dishwasher and washing machine, so I replaced it and the water inlet hoses. Everything was fine for a few weeks until a few days ago when water suddenly started gushing from the Y-piece/hose connection. I've tried undoing it and doing it up several times without luck.
Is there anything obvious I'm missing like sealant tape or a special tool to do it up extra tight?

Aside from not doing up the Y connection tight enough or cross threading, the outlet water connector may be damaged?If my post hasn't helped you, then don't click the 'Thanks' button!0

I fitted one recently and needed adjustable pliers to get it tight enough not to leak.0

No you should not need sealant tape. Are the rubber washers sitting properly in place as you do it up? Sometimes these things do need a little extra mechanical assistance as Norm has suggested but not to the extent where brute force and ignorance is required in large quantities.
Personally I'd plumb them in separately anyway.
