You do NOT need to fill in the whole form, you just need the DS1500 form which your GP or one of the lead MacMillan nurses can do. If you want to claim mobility then you need to fill the parts regarding mobility on the form.

Ask to speak to one of the MacMillan benefit advisors, you'll get better advise than on any forum

So, if you are receiving Radiotherapy, what does that mean? I have no intention of claiming, but would be interested to know before my last session at the end of the month.

See above quoted regulation.
If you are receiving radiotherapy to treat cancer or are likely to do so - which would be the case for an ongoing course - it counts.
Similarly - any recovery time - though that is going to be more of a discretionary time period that the DM may decide.
