Will I be liable to pay tax

I started working for this company 12 months ago in sept 2012, the company put me on a contract for 20k. The first month they did not give me a wage slip. only from the second month they gave me the wage slips. does that mean I have to pay the tax for September myself? I rang the tax office after 3 months and they told me they have not received any details from my employer the tax office then advised me get the pay as you earn reference number from the employer and to ask them to send a p46. I asked the employer for this many times, eventually they gave me a p60 in may 2013, which I sent to the tax office. I rang the tax office last week for my tax refund, but the tax office said they still don't have my employers details on file and that the details I sent to them is at the enquiry stage with the back office. In addition I work as a consultant and the employer transfers extra money into my account to get a car rental every month as i do approx 300 miles per day, the employer also pay for the fuel expense every week into my account when I give them the fuel receipts, I was wondering if i will be taxed on any of this as its all for business use , and will I be liable to pay for tax on this as it is not on my wage slip?? I am really worried as i dont know what to do at the moment, any advice would be great, i have always paid my national insurance and tax in the past. i just dont want to be hit with a high bill from the tax man later on.
