I am currently having to sign on and I have only just heard about this. Does anyone know how I go about applying for one please? I have tried google but it isn't my friend!
I have to have dental surgery and apart from my signing on card, I have no other proof and I'm not sure if they'll accept it.
Help would be appreciated please!

You should have an award letting stating that you are entitled to claim JSA.

I'm not sure about that - for one thing, you have to have INCOME BASED JSA, not contributions based, to be eligible. Assuming that you are in England, I think you really need to get an HC2 certificate....

If you are on a low income you may be eligible to receive financial help through the NHS Low Income Scheme. To apply for an HC2 certificate, you should complete form HC1, which is available from Jobcentre Plus offices or most NHS hospitals. Your doctor, dentist or optician may be able to give you one, too. You can also get an HC1 form by calling 0845 610 11120
