PPI Reclaim - Lombard Direct (Lombard Bank Ltd)

Please can anyone point me in the direction in the correct contact details for PPI reclaim for Lombard Direct. Our loan was taken out in October 2000 and with Direct Line but Lombard bought out Direct Line. I have seen a few threads regarding Lombard but no one set of contact details.
Also, I confess to being a bit confused about the processes of reclaiming too.
Do you need a SAR first or can you just complete the standard form?
In fact, it might be helpful to have the processes bullet pointed!

It's pre regulation so don't get your hopes up.
If direct line sold it to you, then you complain to direct line.
