self employed and SA

so i was self employed until october last year but according to hmrc i still have to complete the return for 2012-13, i no longer have access to the SA section of the hmrc website so need to fill out a form but which 1?

Both.

SA100 is the basic self assessment form

SA103f is the supplement applicable to self employment.
