Guide to completing ESA50 form

I am expecting another ESA50 soon. Had a look on the internet and found this walk through guide:

As the guide is in the format of the actual ESA50 itself, it looks quite useful. Especially to someone who hasn't experienced completing one before.
Any further advice would be gratefully received.
