I bought a 3 month subscription to an online babysitter finder agency. I didn't read the small print in full, and they have since charged me each 3 months (twice), without me realising. In practice I suspect they usually find out when their customer asks them for a VAT receipt (like I mentioned in my previous post).

How HMRC expects companies to know without being told or having a request for a VAT receipt I don't know.
