Change of address proforma for ESA

I am trying to fill out a change of address form that jc+ has sent out...

I have already given all the details already!

There are a couple of questions I'm a little confused about if anyone can help..?

1. Do you live with friends/family as part of THEIR household (Y or N)

I live with my friend who is my carer... But she is not responsible for me, financially !

Further down they want up know the persons income etc.. That is none of my business or there's as we are two desperate entities..?

2. They are asking for my landlords details, I do not want them contacted as people sometimes judge and I don't want them worrying about the rent when there I'd nothing to worry about..

    Who's name is on the tenancy agreement? Yours, your friends or both? If its hers then you live in her household.

    This question is only really relevant if your friend lives with you and you recieve housing benefit, if you don't know thier exact income then just put this on the form

    Again only really relevent if you recieve housing benefit / LHA, but they won't just contact them out of the blue anyway

    This is just a standard form that tries to cover all posssible scenarios do try not to worry about it, but it is important than you return it

    It's a joint tenancy so I'm not in her household...

    These forms are still a little scary with some of there wordings

    You have been incorrectly advised.

    If you don't have a tennancy agreement and you live with your friend then you live with them as part of their household.

    If you do have a tennancy agreement and only your name is on your tennancy agreement (for your room only) then you are not part of your friends household.

    If you have a joint tennancy agreement with your friend, then you are part of your friends household (and they are part of yours) and they must be entered on the form.

    The only time you are not part of other peoples' household is if you live alone, or you have your own tennancy agreement in only your name.

    If you cannot get the private information from your frind then you say to DWP that you have no other relationship than friend and they are not willing to divulge their personal information.... and you see where that gets you.


    The DWP will want to see the tennancy agreement to confirm the above and to confirm the form has been correctly completed. If you want it confirmed how to answer phone ESA.

    You have to give your landlords details, but there should be a box to say that you don't want your landlord to be contacted.
