Hello all,
I was just wondering if anyone knew the answer to this question, on page 4 of the DLA form does my mother put her name and address in the details on that page as she is filling out the form for me? She is not signing the form for me, I can sign my signature. Hope someone can help.

no. someone physically writing the form isnt the same as them 'completing the form for you'0

No need for your mother help. You simply can write it by yourself.0

I often complete forms for friends and neighbours. As long as you are signing the form, your mother just completes the form with your details. HTH
