SAR question for old and closed accounts

Hey there,
Very strangely I received in quick succession two letters from MBNA and Canada Square operations advising me that I had been miss-sold PPI on a credit card and loan respectively.
I was a little surprised to have actively received a letter from these banks, but I suppose that they are likely doing the bare minimum to tick their legal boxes as they certainly haven't made completing the forms in particularly easy.
Unfortunately for me I have two major problems to address.
Firstly, I don't know what what accounts these reclaims related to. I've had two credit cards provided by MBNA in the past 6 years, a more recent BMI Platinum (which I paid off in full every month in order to get air miles) and a Virgin 0% which I had used to consolidate debits. I've similarly had a 0% credit card and a loan with Egg.
Secondly, I don't have any documentation for any of these accounts; largely they were managed on-line, and I now no longer have access to any of accounts as they have all been paid off and closed.
Having done some reading I see that I can send in a SAR to get these account details, my questions are:

- Do I have to explicitly state the account numbers I am applying for, or can I demand information on any and all accounts held currently and previously?
- In the case of the MBNA cards, would I need to send SARs to the respective "branding", or would sending to MBNA be sufficient?
- Finally, if I called the phone numbers given on the forms, would they have to tell me which accounts these forms related to?

- Not in your SAR, but they should have supplied you with the account numbers when they sent the letter but the SAR is asking for all information they hold, not just on one account, but if the informtion has been destroyed, they can't obviously supply it.
