Striking off a Company DS01

I started up a company in 2008.
The company hasnt traded since November 2010. There are no other directors or shareholders except me. No employees either. Its not VAT registered and has no creditors or debtors except me. I put in about £20k into the company via director's loan account. The company has carried forward losses of £15k and share capital of £5k and assets under £100.
I filed a DSO1 form last week. Do I need to send copies to the bank (the account balance is under £5) and HMRC or submit a final set of accounts to HMRC?
ps I cant access the bank account as I cant recall the security details now.

Contact the bank and request a new password
Whilst doing that, ask them to provide you a new copy of the current set of terms.0 -

Wind it down in the process you have said. Are you intending to write down the loss in your own accounts? If so, sit with your or a accountant. It may put money into your pocket0

Send a copy to ALL creditors (ie: those who are owed money) you should have already done this however as you are supposed to inform them of your intention to strike off . . .
So yes send to HMRC and make sure those final accounts are in showing your losses as we would want them objecting to the strike off now would we?!!I was going to put the name of my plumbing business here so you know what I do should I give out any advice plumbing wise - however apparently I cant do that - go figure!!!!!
