Reclaiming PPI through a questionnaire sent by lender

I have been contacted by Canada Square Operations - formerly known as Egg Banking PLC.
They have sent me a questionnaire to complete in case I feel that have been missold my PPI.
My question is, do I use this questionnaire or do I contact them off my 'own back'. I am concerned that they might just think that I am jumping on the bandwagon of PPI claims, when the reality is, I was going to contact them anyway, I just hadn't sent off the questionnaire as I was hoping to find at least one piece of paperwork with a reference number on it in my files, which I hadn't managed yet!
Have any other forumites successfully used a questionnaire prompted by the lender?

Thank you. I realise now it's the same form that I downloaded in January. I guess I'm just worried that they might think that the only reason I'm contacting them is because they've told me I might have a reason to claim, rather than me realising this off my own back, if that makes sense.0

If you have a valid complaint it will be upheld, if not then it won't be. This is my statement as to why I am complaining:
"Although I was aware at the time of my first loan that PPI was being added, I felt pressure to take it out. Being young and financially naïve at the time, I did not contest it too much as I was very much given the impression that it was standard practice to take it out and that not taking it would be a very big risk. Opt-out, rather than opt-in.
When renegotiating the loans in the future, the PPI was added onto my policies as a matter of course with no discussion about whether it was still appropriate. It was only the final time that I consolidated that I *asked* for it to be removed, again it was going to be added without discussion - they were about to process the application when I spoke up."
I am reasonably confident that my claim is valid. Even if they don't agree that I was missold in the first place, the fact that they never discussed the PPI again in the 3 or 4 times I renegotiated the loan is where my main complaint lies.0 -

If your loans formed part of a consolidation "chain", forget about the PPI being added "without discussion" and concentrate instead on checking whether the PPI was added to the loan at the outset so that you paid a single monthly premium for both the loan and PPI. In such cases, extra interest would have accrued as a result. The PPI was added at the outset - I think I was given a figure for how much it was. Unsure as to whether I was charged ongoing interest on it??0 -

A loan on the loan paid to term end with interest, if you cancelled the PPI they would have given you a pro rata rebate of the PPI and the remaining PPI is still on the loan to term end.

You need to restructure your complaint so that it simply focuses on the single premium aspect. You can add that you also feel you were "pressured" into accepting these terms, but better perhaps to say that it wasn't fully explained to you.0
