General PPI Request

Hello to you all,
My sincere apologies if this question has been answered before but I could not find anything similar on here.
I recently received a letter from Nationwide advising I may have been mis-sold PPI. I sent the reply back asking them to investigate and to my surprise they told me I will be receiving a payment of £500 soon. :j
Over the years I have had numerous credit cards but, like Nationwide, I don't recall 'ever' paying for PPI.
So, my query is this - is there a "general" template or request I could send off to these other companies asking them to investigate this for me or do you have to back the letter up with details of when accounts were open/closed etc? (I no longer have any of the old statements).
Any advice would be greatly appreciated.

I was thinking of ringing the companies but now that I have gone through my credit reports I can see that I have quite a few companies to get through and not a lot of details about these accounts so I was thinking it might save time if I wrote to them in the first instance.
If I can get this narrowed down I may give them a ring to find out.
