Best way to correct flat number / finding where wrong one came from?

I just moved and an incorrect flat number has shown up on my equifax report.
What's the best way to fix it? Post / Email / phone / web? Are there any magic words I need to use for it to get through to the right department / make them do the right thing?
I guess I'll likely have to do the same with all three. Will they tell you which company gave them the bad address?
