Reclaiming PPI from RBS

Ok, first of all, there's so many threads it's quite hard to get a grips with it all.
My wife has just got a letter from RBS saying she may be due a PPI claim on 2 loans she had.
First of all, we just got married 3 weeks ago, so should she still apply in her maiden name or married name with footnotes saying we're just married?
now, i take it we'd be better using the template from off of here instead of using the letter template from the RBS so it will take it away from an RBS sided letter/investigation and make it more formal
cheers for any pointers or links posted to get us started on this, as this could pay for or some our honeymoon
