    They only ask for the NI number for people claiming an exemption of prescription fees as a result of being in receipt of Income Support, JSA IB, Pension Credit or with a NHS Tax Credit Exemption Certificate.

    That said, I get my prescriptions free because of Tax Credits and I have *never* been asked to fill that in. I only ever tick the right box and then complete part 3.

    You also don't need to sign for your own items. You can send someone to collect on your behalf and they can sign as your representative.

    A representative can sign for any prescription though no?

    You can even make up an NI number

    Thats true,

    but I have a condition that is covered by the Equality Act 2010, but I put my NINO on the forms because I know it !.
