Just make it up as best you can. They can't check. I had a form from a hospital like that. I knew the months but not the dates but it was on the web and wanted the dates so I said I started each job on the first of the month and left on the last day of the month. It was fine, I got an interview and nobody questioned it.0

Id never lie that's why I have sent them an email for advice. If they say they need all the dates and I mean precise dates then I cant apply.Just make it up as best you can. They can't check. It's not a lie if you don't know the exact dates. It's an approximation. Write that on the form if necessary. I have to say that if anyone wrote to me asking such a question I'd assume they had no initiative and would mark them down for it.0 -
