Complained via email to CEOs at ASDA and N/wide Building Society :
ASDA...Selling wine with a competition ticket that was out-of-date. Asda sent me a £10 gift card so I got the wine effectively for free and am £5 in profit.
N/wide BS...Problems closing one of my accounts. Credited my account with an extra £25 before closing it as compensation.
You just can beat escalating via email to CEOs

With or without escalation (which rarely actually goes to the CEO) both your above items come within the terms of the 'standards of service' published by the companies you mention.35, semi retired, sun, sand, sea, life is good

NHS websites' complaints page was great for getting action, when i worked for an firm chasing unpaid agency worker bills!Long time away from MSE, been dealing real life stuff..

But this method of complaining would be flawed if everyone did it. The CEO in most cases won't have read it someone else would have a "director" of something or other. Then if every single person did this you wouldn't keep getting complaints resolved at director level, they'd all just get forwarded to the main customer service department and dealt with by anyone but probably with something like "head of customer services" at the bottom of each reply just to make it look like it went somewhere important.0
