DLA GP's report.

When a GP is asked to send a medical report about a patient's claim for DLA does he:

Send all the patient's medical notes
or
Fill out a form sent to him by the DWP
or
Receive a copy of the patients DLA claim form and comment on it?

I just wondered if every GP sends in the same form as it would be very unfair if some people lost the chance of DLA because of the brevity of their doctor's report.

    I applied for DLA and was turned down i asked for DWP to send me the doctors report i was just curious as to what the doctor had wrote about me .The report looks like a standard letter which they send every doctor.

    It was basically a A4 sheet of paper saying to your knowlegde does your patient need help at doing this or that .There was small boxes for the GP to put there input .

    Hello.

    DLA have a new (ish) medical report wizard thing whereby the questions that are asked are geared to the disability or condition. For instance, a GP who's patient has mental health problems will be asked about things like 'do they self neglect or would they if they lived alone?' rather than 'what is their exercise tolerance?'

    There are around 8 questions in total mainly geared around the specific disability or condition which makes much more sense really. It's easier for the GP to complete rather than put 'I don't know' and it's easier for the DM to get a clearer picture of a customers needs.

    Some GP's may occasionally attach letters from the hospital or specailist if they think it will help a persons claim but it's rare for them to just send notes.
