HELP - Stock Transfer Form ?

Hi All
Transferring my shares from Selftrade to a cheaper provider, i have downloaded the transfer form but confused on the -
FORM of CERTIFICATE REQUIRED WHERE TRANSFER IS EXEMPT TO STAMP DUTY.
It is not an ISA just a standard share dealing account.
Anyone help ?
This is the form, i hold ordinary shares of 0.1p, 0.5p, 10p, and 15p and have inputted that into the top of the form plus the number of units held in figures and written.

Are you sure that you have the correct form? The one being referenced is for transferring shares that are held as paper certificates, i.e. you have direct ownership of them. If the shares are still held in SelfTrade then they are likely to be in their nominee account, in which case you would need to use a different form unless you have had them converted to paper certificates.
Assuming that you are looking to transfer to X-O, and that your shares are current;y held in SelfTrade's nominee account, then the form to be used can be downloaded via http://www.x-o.co.uk/how_to_use.htm#6 Looks like everything has to be transferred, though, rather than being able to be selective - so long as that is OK.Living for tomorrow might mean that you survive the day after.
