Life assurance in trust question

Looks like L&G are the cheapest insurer for our joint policy which we want to write in trust so that it doesn't form part of our estate.
I've downloaded the application form and all I can find on the form is a question asking if it is the intention to put the policy under trust. So I tick that box... and then what?
Will L&G automatically send me another form to fill out or do I need to be doing something else?
An application form I looked at from another insurer actually included the trust section to fill out, hence my confusion.

L&G will do nothing unless you tell them. There should be a trust form and additional trustees form also available to download. There are multiple trusts so make sure you get the right one for your circumstances.I am an Independent Financial Adviser (IFA).
